Chapter Dues Invoice Request - Student National Dental Association

Thank you for your interest in becoming a National SNDA Member!

This form is for multiple Student Doctors from the same dental institution to pay their dues in a single transaction. It is especially useful for institutions that pay dues on behalf of its Student Doctors. It’s as easy as 1, 2, 3!

Download the excel file “SNDA 23-24 Chapter Dues Demographic Information Form” below and enter the demographic information for each student to be included in the group payment. You must clink the words “SNDA 23-24 Chapter Dues Demographic Information Form” for the file to download.
Once the excel sheet is completed in entirety, you will be ready to submit this form.

The price for SNDA National Membership dues are as follows:

15 September 2023- 15 December 2023: $60 per Student Doctor

After 15 December 2023: $85 per Student Doctor
